A full diagnostic of how the organization actually works — combining 24 months of incident data, systemic causal analysis, SMS benchmarking against EPRI and OSHA VPP standards, structured interviews, and multi-site field validation. Everything in the SGA, plus the data science and executive-grade reporting to back it up.
5-Phase Methodology
Wk 1–2
Phase 1 · Preparation & Culture Survey
Kickoff meeting, scope alignment, and data collection — incident records, outage data, and safety reports. Includes administration of the REMEDY Culture Survey to establish an organizational performance baseline across executive, leader, and employee levels.
Wk 3–6
Phase 2 · Incident & Data Review
RCA techniques (5-Whys, Fishbone), Pareto trend analysis, and systemic causal mapping across 24 months of recordable incidents, MVAs, outages, and near misses. Investigation process gap evaluation included.
Wk 7–9
Phase 3 · Program Review
SMS assessment benchmarked against EPRI guidelines and OSHA VPP standards. Covers Observation Program, JSA/Pre-Job Briefing, training systems, safety communication, and leadership frameworks. 20–30 structured interviews.
Wk 10–11
Phase 4 · Field Assessment
2–3 days per location across selected job sites and active project areas, with site selection driven by incident data trends. Assesses culture, leader behaviors, and field practices — informed by findings from Phases 2 and 3.
Wk 12–14
Phase 5 · Reporting & Recommendations
Prioritized implementation roadmap with success metrics. Weekly updates, mid-project preliminary summary at Week 7, and final executive close-out presentation.

Stratified by incident type, location, and severity — 100% review of high-severity events; 50–70% of low-severity for trend identification.
Interviews
20–30 targeted interviews across executives, supervisors, and field workers for a full vertical cross-section.
Field Sites
Sites selected based on incident data concentration and trend analysis — not random selection.
